We are seeking a dynamic candidate to join the Primary Health Solutions team as a Referral Specialist at our Administration office. This role is key in connecting patients with specialty providers in order to deliver comprehensive medical care. The Referral Specialist is responsible for guiding the patient through the referral process to ensure that the patient receives the diagnostic tests or specialist consultation as ordered by the provider. The referral specialist along with other members of the Patient Access Team work together to assure that the patient documentation and consultation notes are returned to the patient chart and reviewed by the clinical team.
A day in the life of a Referral Specialist:
- Collect and analyze referral source information in order to match and refer patients to specialists and entities that accept the insurance payors of the patients served by Primary Health Solutions.
- Develop a list of specialist providers accepting new patients, and those who are sensitive to the specific needs of Primary Health Solutions patients.
- Use electronic referral tracking software to complete and track referrals.
- Contact the referral source to provide appropriate patient information.
- Communicate with the specialty entity electronically, sending referrals and supporting documentation to complete the visit.
- Document transaction in PHI log of the patient Electronic Medical Record.
- Contact patients to provide referral information and answer questions.
- Assist in closing referral loops after documentation is received back from the specialty encounter.
- Follow up on incomplete referrals.

Requirements
Success Requirements:
- Minimum of a high school diploma or equivalent required.
- Graduation from an accredited Medical Assistant program preferred.
- 2-3 years of experience in a clinical setting.
- Read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.
- Write clear and concise routine reports and correspondence.
- Solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
- Interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
- Knowledge of current practice management system, electronic medical record, Microsoft Word, Internet, and Intranet. Must also be proficient in Microsoft Word, PowerPoint, and Excel.
- Certified or Registered Medical Assistant or valid Ohio nursing license if applicable.
- Ability to speak Spanish is desirable.
- Excellent communications skills, both written and verbal.
- Ability to work effectively and independently with all levels of clinical and administrative staff within the health centers and with community providers.
- Knowledge of examination, diagnostic and treatment room procedures. Knowledge of medical terminology.
