As a clinician with PRN HH&T you will treat patients in the SNF setting with opportunities to train in the home health setting in Fond Du Lac and the surrounding areas

Job Duties include:
- Evaluate and develop individualized patient treatment plans
- Work with clinical staff, patient and families to maximize patient’s use of functional abilities
- Document the course of treatment in a clear, concise manner, including a discharge summary utilizing the prescribed format.
- Instruct Physical Therapist Assistants in treatment procedures to be utilized and provide supervision
- Collaborate with clinical team to develop and educate patient and/or care givers on safe discharge plan and/or care givers on safe discharge plan.
