Objective of Position: Coordinate, facilitate and monitor quality services provided to all clients served at Quality Life Services.
Quality Life Services LLC is seeking a full-time Service Coordinator. The Service Coordinator assists adults with developmental disabilities to participate as active and included members of their community and help them realize their personal goals.
The Service Coordinator is responsible for:
Managing a caseload, Planning, organizing and implementing DDW services to developmentally disabled individuals. Interpreting and implementing DD Waiver standards and regulations. Working as part of a team to ensure client health and safety and provision of appropriate services. Maintaining complete and correct documentation in accordance with DDW Standards. The ideal candidate is someone who: works well with teams. Is highly detail-oriented and organized. Demonstrates sound judgment and decision-making. Is accountable and able to work independently with minimal supervision. Has an associate's degree (or above) and/or prior DDW experience.
Knowledge, Skills and Abilities:
- Assure accurate documentation for all billable services; timely and accurate implementation of Individual Service Plan (ISP).
- Coordinate client’s annual ISP's in consultation with the individual, and his/her team and in accordance with person centered planning guidelines.
- Possess and have the ability to implement organizational and leadership skills.
- Ability to manage multiple priorities and work under stressful situations.
- Organize and manage caseload to ensure all annual ISP's, required ISP reviews, IDT's, annual, or quarterly reports are conducted/documented in a timely manner.
- Maintain basic computer skills to include working knowledge of Microsoft Windows (primarily Word, Excel, and Outlook), current data databases. Knowledge of Therap is a plus.
- Must have a valid driver's license.
- Maintain vehicle insurance as per state law where residing. Possess reliable transportation and willing to transport clients when required.
· Ensure all reports are complete within policy timeframe
· Provide advice and solutions to staff through QLS on call.
· Create and maintain all QLS documents.
· Notify Guardians and Case Managers in case of emergencies or ANEs.
· Notify staff of interviews with DHI.
· Review and approve all monthly documentation.
· Review and refer to established DOH policies and procedures when needed.
· Other duties may be added if needed.
Educational Requirements and Experience:
- Have a high school diploma or G.E.D, Associate in Business, Social Work, Human Service or related field, one year supervisory experience and one year experience working with individuals with disabilities or a combination of education, specialized training and experience totaling three years of which one must be in a supervisory capacity.

Pay: $16.00 - $17.00 per hour
