Community Living Specialist

Job Description

The Community Living Specialist is in constant face-to-face contact with persons served primarily in a Residential setting but may also be assigned tasks and functions in a Day program setting covering vacancies or absences as required. The Community Living Specialist’s goal is to increase the persons’ capacity for independent community living, through the establishment of support, rapport, role-modeling, teaching, and the provision of activities and learning experiences. The Community Living Specialist consistently exercises discretion and sound judgment in the performance of the duties of the position.

TASKS AND DUTIES:

· Personal Hygiene/Health

· Housekeeping

· Money Management

· Transportation Skills

· Performs Activities of Daily Living (ADL) management and supervision. May perform ADL functions where necessary including cooking, cleaning, laundry, banking, transporting, or shopping, etc.

· Performs medication monitoring.

· Assists persons served to become independent and to encourage efforts towards recovery.

· Documents all interactions, observations, assessments, plans, etc. Completes paperwork and reports as assigned.

· Performs crisis assessment, intervention, and management.

· Performs other duties as assigned.

MINIMUM QUALIFICATIONS:

EDUCATION & EXPERIENCE:

High School Diploma or GED; plus, one (1) year adult mental health experience required. Preferred - Associate Degree (Certificated Nursing Assistance (CNA), Medical Technicians (Med Tech.) /or related certified professional. College degree may replace experience requirement.

GENERAL REQUIREMENTS, SKILLS, & ABILITIES

Must be at least 21 years of age and possess a valid driver’s license. Must present credentials that are current and valid and meet safe driving requirements. Must submit to a driving record and criminal background screening. Must be capable of satisfactorily performing all job functions and tasks with minimal supervision. Must be able to maintain the highest level of ethical standards and professional relationships. Must possess excellent oral and written communication skills, including an excellent command of the English language.

Job Types: Full-time, Part-time

Pay: Up to $14.00 per hour

Education:

  • High school or equivalent (Preferred)

License/Certification:

  • Driver's License (Preferred)
