Health Coach Overview:              

Under the general direction of the Health Care Home Director and Health Club Program Manager, the Health Coach provides health and wellness coaching to people in CPRC services. This role requires proactive multitasking and coordination. The Health Coach must embrace ReDiscover’s aspirational culture of respect, excellence-focused, dedication, inclusion, strategic, compassionate, optimistic, visionary, engaging and recovery-driven.

Strategy & Planning

• Provide instruction, coaching, and encouragement to those served to achieve desired health and fitness goals.
• Provide oversight of the exercise program on-site.
• Develop and maintain relationships telephonically, and in the community with contracted gyms and other community partners.
• Outreach to referred clients to join the Health Club.
• Collaboration with the mental health team to ensure consistent treatment.
• Monitoring, maintaining, and entering data related to the client(s) physical health.
• Other duties as assigned.

Operational Management

• Good written, oral, and interpersonal communication skills.
• Acts with integrity by observing ReDiscover policy/procedures, accreditation standards, laws and ethics.
• Supports the strategic goals and mission/vision/values of ReDiscover when interacting with clients, co-workers and community stakeholders
• Contributes to the mission of ReDiscover by adjusting work priorities and duties as needed to meet unforeseen demands.

Qualifications

By joining Team ReDiscover, our expectation is:

• Bachelor’s Degree in a Social Science, health and wellness, or a related field from an Accredited College or University.
• Certified personal trainer, nutritionist, or dietician preferred.
• Valid Missouri driver’s license. Must meet guidelines of the agency’s driving criteria (i.e. Proof of liability insurance) and maintain the criteria as per company policy at all times.
• Understanding the function of nonprofits, specifically within behavioral healthcare is preferred.
• Strong analytical capabilities and conflict-resolution skills.
• Intermediate computer skills in typing/keyboarding, Microsoft Word and Excel programs as well as G-Suite.
• Self-Driven individual with a customer service focus.
• Team-Oriented and skilled in collaborating with others.
