ReDiscover is looking for a Nurse Care Manager to add to our in-clinic Health Care Home Department!

WHAT WE DO
Healthcare Home is a population health management program designed to champion healthy living changes and promote preventative care, health education, and healthy activities for clients, staff, as well as the community.
At ReDiscover, while every day is different, a typical day may include the following roles and responsibilities:
- Complete health and metabolic screenings on a monthly, quarterly, and yearly basis.
- Work collaboratively in an interdisciplinary team to develop treatment plans.
- Provide health and wellness education for clients and staff.
- Facilitate educational groups tailored to clients’ needs and interests.
- Manage and collect medical information on clients’ hospital admissions and discharges.
- Develop working relationships with internal and external providers involved in the client’s treatment plan.
- Review and monitor performance measures and outcomes on clients’ physical health.
We are focused on your development and will spend time helping you continue to grow as a Nurse Care Manager. However, here are some that you will need prior to starting with ReDiscover:
- Graduated from an accredited nursing program
- Active Missouri license as an LPN or RN
- Two (2) years of nursing experience in the mental health setting is preferred.
- Working knowledge of mental illness and substance use disorders
- Excellent customer service skills with a recovery focus
- Strong written, oral, and interpersonal communication skills
- Intermediate computer skills in typing/keyboarding, Microsoft Word and Excel programs as well as GSuite
