ReDiscover is looking for a Nurse Care Manager to add to our in-clinic Health Care Home Department!

WHAT WE DO
Healthcare Home is a population health management program designed to champion healthy living changes and promote preventative care, health education, and healthy activities for clients, staff, as well as the community.
At ReDiscover, while every day is different, a typical day may include the following roles and responsibilities:
- Working collaboratively in an interdisciplinary team to development treatment plans centered around client’s wellness.
- Completing health and metabolic screenings for clients on a quarterly or yearly basis depending on needs.
- Building rapport with clients by providing quarterly outreach calls to engagement.
- Providing health and wellness education for clients and staff.
- Facilitating educational groups tailored to client’s needs and interests.
- Managing and collecting medical information on client’s hospital admissions and discharges, and ensuring that medication reconciliation is completed after discharge.
- Developing working relationships with internal and external providers involved in the client’s treatment plan.
- Reviewing and monitoring performance measures and outcomes on client’s physical health.
Qualifications
We are focused on your development and will spend time helping you continue to grow as a Nurse Care Manager. However, here are some things that you will need before starting with ReDiscover:
- Graduated from an accredited nursing program.
- Active Missouri license as an LPN or RN.
- Two (2) years of nursing experience in the mental health setting preferred.
- Working knowledge of mental illness and substance use disorders.
- Excellent customer service skills with a recovery focus.
- Strong written, oral and interpersonal communication skills.
- Intermediate computer skills in typing/keyboarding, Microsoft Word and Excel programs as well as G-Suite.
- Ability to learn and utilize ReDiscover’s electronic health record.
- A valid automobile driver’s license, appropriate automobile insurance and a safe, reliable, registered automobile available for use during work.
