CASE MANAGER I and II (Bilingual Spanish)

Case Manager I Bilingual - $2,590.36 - $3,769.85 Bi-weekly, $67,349.36 - $98,016.10 Annual

Case Manager II Bilingual - $2,689.15 - $3,915.81 Bi-weekly, $69,917.90 - $101,811.06 Annual

The Regional Center of the East Bay (RCEB) is one of 21 Regional Centers that are community-based, private non-profit corporations under contract with the California Department of Developmental Services. RCEB provides intake, assessment, diagnosis, and coordinates community-based services for over 25,000 children and adults with developmental disabilities in Alameda and Contra Costa counties. 

RCEB envisions a future where persons with developmental disabilities are truly a part of the community. Persons with developmental disabilities receive supports and services they need to succeed. The supports and services change as the need changes.

In our dream, children with developmental disabilities live with families in typical communities. Many choose to attend their neighborhood schools and play with neighborhood friends. Adults with developmental disabilities live in the residence of their choice – with family members, with friends, or with their spouse / significant other. They engage in activities of their choice – work, volunteering, education, training, or leisure. They have meaningful relationships with friends and coworkers and are seen as valuable, contributing members of their communities.

 

Key Responsibilities: 

Case Managers are an integral part of realizing our dream. Under general direction, Case Managers are responsible for the entire case management process, including: assessment and evaluation, planning, authorizing, advocacy, monitoring, reviewing documentation for completion.

  1. Facilitate the planning process and develop the Individual Program Plan (IPP) or Individual Family Service Plan (IFSP) for assigned Consumers and oversee the implementation of the IPP/IFSP. Monitor and evaluate the services provided through the plan.
  2. Coordinate and support the implementation of the Consumers plan which may include identifying natural supports and generic services and evaluating when a need exists that can only be met through RCEB authorized funding.
  3. Advocate on behalf of consumers to protect their rights under the law and ensure that they receive those services and benefits to which they are entitled.
  4. Assist the consumer and family in obtaining essential diagnosis and evaluation from appropriate practitioners and secure written reports and other information which assesses and interprets the consumer’s situation.
  5. Monitor the consumer’s health and safety.
  6. Maintain all documentation and contacts as required by law, regulation and policy.
  7. Implement RCEB policies and procedures.
  8. Complete, in a timely manner, functional and/or developmental reviews.
  9. Keep supervisor informed of the status of the case load and unmet needs in the community.
  10. Perform other duties as assigned.

Minimum Qualification:

Case Manager I

  1. BA Degree in Social Work, Psychology or a related human services field and 1 year similar experience. B.A. in an unrelated human services field plus 4 years of experience working directly with the developmentally disabled in progressively responsible positions, may be substituted for a bachelor's degree in the human services field.
  2. Ability to communicate clearly and effectively, verbally and in writing.
  3. Automobile and valid driver's license required.
  4. Demonstrate cultural awareness and sensitivity and an ability to work effectively with culturally diverse populations.
  5. Ability to speak and read in Spanish.

Case Manager II

  1. Master’s Degree in Social Work, Psychology or related human services field.
  2. Ability to communicate clearly and effectively, verbally and in writing.
  3. Automobile and valid driver's license required.
  4. Demonstrate cultural awareness and sensitivity and an ability to work effectively with culturally diverse populations.
  5. Ability to speak and read in Spanish.

RCEB staff are mandated to be vaccinated and receive a booster, when eligible, for COVID-19, by 3/1/2022, per CA Public Health Order (PHO) and DDS directive. *The Public Service Loan Forgiveness program is a federal program that forgives student loan debt for nonprofit employees after 120 qualifying payments. Employees who work at least 30 hours per week for a governmental, 501(c)(3) nonprofit, or other qualifying nonprofit organization may apply to have the balance of their student loans forgiven (tax free) after making 120 monthly payments.
