Hours and Benefits:
- 40 hour work week, Monday through Friday 8am-4:30pm with some flexibility

Requirements for the Care Manager:
- Bachelor’s Degree in Human Services or a related field or a CASAC along with 2 years of experience in the Human Services field.
- Valid NYS Driver’s License required.
- Successful clearance by the Justice Center and New York State Office of Children and Family Services Statewide Central Register Database Check also required.
Must posses the following skills:
- Ability to work independently and also be highly dependable
- Strong written, verbal and computer skills
- Extremely organized and demonstrate an ability to carry out multiple ongoing tasks with the ability to prioritize
Responsibilities for the Care Manager include:
- Assist high needs individuals returning to the community from prison identified by the County Reentry Case Review Committee and to coordinate with the Reentry Coordinator and parole.
- Provide linkage to services, collaborate amongst community providers, and coordinate care for everyone with the goal of reducing recidivism of re-arrest and parole violations, and increase integration into the community.
- Responsible for engaging in pre-release planning, conducting initial assessments, developing service coordination plans, enrolling in employment readiness and Cognitive Behavioral Intervention programs, and participating in case conferences.
