In this role you will….
Provide supervision to the Individual Support Workers, you must be available on call-duty as needed. Assists clients with daily living activities in an individual’s home or residential facility. Observes and monitors client’s behavior and charts observations and incidents. Provides training in basic personal and social skills, simple academics, or work skills. May assist with meal preparation, ADLs, and provide transportation.

Responsibilities:
Ensure that goals are tracked in the home.
Attend ISP meetings (annually, at day center)
Weekly home meetings
Facilitate completion of monthly safety drills and turn in quarterly to office.
Training of direct support staff in home
Financial management of individual funds
Instructs and advises individuals on daily living skills in one-on-one or group instruction.
Observes and monitors client behavior.
Counsels and advises clients on behavioral problems, daily decision-making, and the resolution of minor problems.
Provides personal care as needed.
Performs housekeeping duties, such as cleaning, washing clothes and dishes, and running errands, as needed.
May plan, shop for, and prepare meals, including special diets. (Weekly grocery shopping).
Provides transportation for clients/residents to appointments and activities; to include doctor appointments as needed.
Upkeep of the agency vehicle assigned to the home.
Maintains that medications are current in home.
Monitors self-administration of medications.
Reports home repairs
Flex schedule to cover staff call outs, holiday coverage as needed.
Technical Competencies:
Ability to observe and monitor participants’ behavior.
Ability to assist and supervise participants with daily living activities.
Knowledge of agency confidentiality policies.
Knowledge of individualized service plans.
Knowledge of the documentation process.
Ability to communicate both orally and in writing.
Ability to plan recreational activities for participants.
Knowledge of agency safety policies and procedures.
Ability to complete all daily housekeeping chores.
Ability to identify and report facility repairs and equipment problems.
Conflict resolution skills.
Ability to identify disruptive behavior.
Ability to train individuals in personal hygiene and other daily living activities.
Ability to recognize contributions and celebrate accomplishments.
Other duties as assigned.
Requirements:
HS diploma/ GED
Valid Driver’s License with reliable transportation
Acceptable Driving record
Acceptable background check
Must pass the drug screen.
Current physical/ TB test
Acceptable references.
Ability to pass basic CPR and first aid.
1-year direct support experience.
