Responsibilities:
• Conduct client intakes, perform assessments, and create comprehensive case plans tailored to individual needs.
• Provide crisis intervention, counseling, and practical support to help clients resolve urgent issues related to housing, income, transportation, or health.
• Monitor client progress, advocate for their needs, and assist with administrative tasks such as form completion, translation, and correspondence.
• Maintain accurate and organized client records, both physical and digital, while ensuring data integrity in relevant systems.
• Support front desk operations, including handling inquiries and generating statistical reports to evaluate program outcomes.
• Address client concerns and provide guidance to staff and volunteers, fostering a collaborative and supportive environment.
• Represent the organization professionally in interactions with clients, external agencies, and community partners.
• Attend required trainings, meetings, and adapt to flexible schedules, including evenings, weekends, and holidays.
• Report incidents promptly and adhere to organizational policies and procedures.
• Recommend improvements to case management processes to enhance service delivery and effectiveness.
• Bachelor’s degree in Social Work, Psychology, or a related field; Master’s degree in Social Work preferred.• Minimum of 2 years of experience in case management or similar roles, with both administrative and practical responsibilities.
• Familiarity with community services and resources for economically disadvantaged populations, including inter-agency collaboration.
• Experience supporting individuals facing co-occurring challenges such as mental health issues, substance abuse, homelessness, or trauma.
• Proficiency in Microsoft Office and ability to learn systems like Clarity Database; familiarity with telecommunication tools like Zoom.
• Bilingual in English and Spanish or Cantonese/Mandarin is highly preferred.
• Strong analytical, communication, crisis management, and organizational skills.
• Reliable transportation and a clean driving record, with the flexibility to work evenings, weekends, and holidays as needed.
