What are the responsibilities and job description for the Direct Support Professional position at Rockland Hospital Guild Clue?
The role of the counselor is to provide in person support to adult individuals who suffer from a severe and persistent mental illness and reside in our treatment apartment program on-site. Residence Counselor duties are as follows: Assist residents with their service plan goals which include the following- daily living skills, medication management, health services, symptom management, community integration, substance abuse, and anything else they may feel is necessary for their independence. Must be able to contact providers as needed and request needed documentation. Must be able to scan, fax, e-mail, write service plan or progress notes as needed.
Hours will vary, facility open 24/7. Shifts are 8am-4pm, 4pm-12am, 12am-8am
ASL proficiency a plus
*Signing bonus $250 (after 3 months)
* Quarterly bonus 10% (funding permitting but right now it’s through 2022) *yearend bonus (funding permitted)
* Part time sick pay
**Weekend and evening availability a must**
