This position will work at 6043 Hollywood Blvd. Schedule is 4 days a week/10 hrs a day. 3 days in office, 1 day remote.
Duties and Responsibilities:
- Responsible for coordinating with those individuals and/or entities to ensure a seamless experience for the member and non-duplication of services
- Oversee provision of ECM services and implementation of the Care Plan
- Offer services where the member lives, seeks care, or finds most easily accessible and withinManaged care plan (MCP) guidelines
- Connect member to other social services and support needs, such as transportation and housing.
- Advocate on behalf of members with health care professionals
- Use motivational interviewing, trauma- informed care, and harm-reduction approaches
- Coordinate with hospital staff on discharge plan
- Accompany member to office visits, as needed and according to MCP guidelines
- Monitor treatment adherence (including medication)
- Provide health promotion and self-management training
- Inform patients about other services offered by Saban Community Clinic that may be appropriate for the patient’s family members.
- Performs other duties as assigned by Supervisor.
Position Requirements:
- Bachelor and/or master’s degree in social work or related fields or equivalent experience
- 3 years prior case management experience
- Valid California driver’s license and insurance
- Related work experience in public health, health outreach and education
- Fluency in another language such as Spanish, Russian or Korean preferred.
- Covid vaccination and booster
