Title: Community Outreach Coordinator

Department: HICAP – San Mateo

FLSA Status: Non-Exempt

Reports To: Program Manager

Summary: Responsible for promoting greater awareness of Medicare benefits and HICAP services to Medicare beneficiaries and their families throughout San Mateo County.

Essential Functions:

  • Promotes greater awareness of HICAP services to Medicare beneficiaries and their families throughout San Mateo County; develops and implements an outreach work plan in collaboration with the Program Manager.
  • Responsible for coordinating outreach events and presentations with local businesses, community partners, civic clubs and congregations. Provides information and promotes utilization of HICAP services by maintaining an updated media kit. Identifies potential partners in media including special and minority media to reach underserved communities.
  • Distributes Medicare and related outreach materials during outreach events; identifies the need for additional outreach materials in target communities (e.g. brochures, informational pamphlets, press releases, announcements, flyers, posters and signs) and assists with their development in appropriate languages.
  • Maintains a list of volunteers who assist with outreach events and coordinates their help as needed; works with the Volunteer Coordinator to accomplish tasks.
  • Recruits and trains registered counselors to become Community Educators, along with the Program Manager, who meet the required criteria with the California Department of Aging (CDA). Responsible for overseeing their internship phase 1 & 2, documenting their internship hours, and inputting information in SHARP. This includes but is not limited to on-site observations of Community Educators at least once a year and technical assistance as needed with creating PowerPoint presentations.
  • Enters outreach events “Public and Media Activity Reports” in the SHARP system required by CDA by the 15th of each month, and keeps monthly records in accordance to program protocol.
  • Participates in outreach roundtable discussions, relevant workgroups and training sessions as assigned by the Program Manager; attends community meetings and reports back to staff and volunteers so as to better promote our services.
  • Performs all duties necessary to assist the Program Manager in ensuring program compliance with its contractual obligations regarding the outreach component of the program.
  • Substitutes for registered volunteer counselors and community educators when needed.
  • Performs other duties as assigned.

Qualifications:

  • Bachelor’s degree or equivalent training with at least two years of experience working in senior and disability services. Experience in outreach and marketing is highly preferred.
  • Must complete HICAP counselor training and/or registration within 6 to 12 months of employment, depending on program needs.
  • Knowledge of Medicare, Medi-Cal, MediGap, managed care, long-term care insurance and health care delivery systems preferred.
  • Excellent verbal and written communication and interpersonal skills; and experience in group presentations. Bilingual in English and Chinese/Spanish preferred.
  • Sensitive to the needs of the elderly and adults with disabilities from diverse ethnic and minority cultures.
  • Ability to work independently and in a team environment; lift or pull 25 lbs.
  • Proficient in MS Office and the Internet.
  • Must have and maintain a valid CA driver license, satisfactory driving record, and auto insurance coverage as specified in Self-Help’s policy.
