The Program of All-Inclusive Care for the Elderly (PACE) offers a variety of services, with many of them provided onsite at a PACE Center.
PACE is an alternative to nursing home care and – through an interdisciplinary care team (IDT) of physicians, nurse practitioners, nurses, social workers, therapists, van drivers, and aides – coordinates and provides preventive, primary, acute, and long-term care services, so older individuals can continue living in the community.

Primary Purpose
To lead centralized services throughout the organization including transportation, clinic scheduling, reception and. The Associate Director of Centralized Services will collaborate closely with other leaders to ensure smooth and efficient services are being delivered. The Associate Director of Centralized Services will set the vison and culture of the department. The Associate Director of Centralized Services will function as an important member of the team and in line with essential foundational Program of All-inclusive Care for the Elderly (P.A.C.E.) concepts including:
  • Comprehensively supporting natural caregivers and/or family members.
  • Respecting and honoring participant health care choices.
  • Creatively promoting participant independence and autonomy.
  • Uniquely understanding and addressing behaviors related to participant non-compliance.
  • Delivering the highest quality of care.
Key Responsibilities
  • Assures the mission, vision and core values for Senior Care Partners PACE are adhered to, in accordance with organizational strategic plan and assists in guiding long-range objectives.
  • Work closely with the leadership team to create a vision and culture for department and be a role model for this vison and culture.
  • Collaborates with other leaders to establish and maintain consistent care delivery for participants.
  • Collaborate with others to create and develop standard processes.
  • Work closely with the quality department to create and lead the team in quality benchmarks.
  • Work closely with outside vendors to maximize the efficiency of services.
  • Assists with the development of budgets and identify staffing to meet participant needs.
  • Leads and facilitates meetings within the organization and with outside vendors.
  • Manage and approve payroll and PTO requests.
  • Participates in setting and achieving organizational benchmarks and performance goals.
  • Leads and promotes a high level of customer service to our participants, visitors, and internally between departments and staff members.
  • Ensure compliance with all Center for Medicare and Medicaid Services (CMS) and Michigan Department of Health and Human Services (MDHHS) guidelines and regulations.
  • Monitors resources closely and implements initiatives to ensure operational efficiency, staff productivity, and cost effectiveness of services.
  • Participates in applicable community initiatives and workgroups as an expert in PACE.
  • Work with contracts to find new transportation contracts.
  • Responsible for successful onboarding of new transportation vendors
  • Participates in the participant grievance process.
  • Responsible for assisting Human Resources (HR) with new hires to include employee selection, pay determination, roles and responsibilities, and FTE management.
  • Utilize the Entrepreneurial Operating System business model to drive focus, discipline, and accountability for your team.
  • Prepares periodic reports to review staff performance as related to planning, caseload allocation, and quality of work, procedures, and patient care outcomes.
  • Empowers employees and teammates by sharing responsibility, sets and communicates attainable expectations, objectives, and goals for others, and supporting a spirit of commitment, ownership, and teamwork.
  • Conducts performance evaluations and disciplinary action for direct reporting staff.
  • May be asked to serve as the communication liaison with participants, family, and provider network as needed.
  • Responsible for coordinating necessary in-services and educational opportunities for direct reporting staff/departments and newly hired staff.
Leadership Responsibilities
This position has leader responsibilities:
  • Transportation Scheduler, Referral Coordinator, and Receptionist
Qualifications
  • Bachelor’s Degree required or any combination of education and related work experience, master’s preferred.
  • Minimum of 5 years of related experience with at least 2 years leadership experience.
  • Must have proven leadership experience and skills with the ability to get along harmoniously with others in a team environment.
  • Demonstrates ability to effectively facilitate meetings.
  • Demonstrates ability to effectively build team cohesion.
  • Strong communication skills, both written and verbal.
  • Demonstrates the ability to educate and empower others.
  • Must possess the ability to plan, organize, develop, implement, and interpret goals, objectives, policies and procedures necessary for providing quality care.
  • Must have patience, a cheerful disposition and enthusiasm.
  • Well-developed ability to show discretion and maintain confidentiality.
  • Proficiency with MS Office Suite, Word, Outlook, PowerPoint & Excel.
  • Minimum of one year of experience working with the frail, elderly, or long-term care population.
Specific Requirements
  • Minimum of one year of experience working with the geriatric population.
  • Demonstrate our core values of compassion, integrity, teamwork, and excellence.
  • Possess current CPR certification by AHA, or willing and able to obtain as offered by Senior Care Partners PACE.
  • Must be knowledgeable of current medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long-term care.
  • Must be willing to seek out new methods and principles and willing to incorporate them into existing medical practices.
  • Must possess the ability to make independent decisions when circumstances warrant such action.
  • Must possess the ability to communicate tactfully with personnel, participants, family members, visitors, government agencies/personnel, and the public.
  • Must possess personal qualities of integrity, credibility, and commitment to Senior Care Partners PACE's mission.
Work Environment
  • Has a visible presence while working in the center and is subject to frequent interruptions.
  • Has a working presence in the community and is involved with participants, family members, personnel, visitors, government agencies/personnel, etc., under all conditions/circumstances.
  • Works beyond normal working hours and on weekends and holidays when necessary.
  • Participates in an on-call rotation
  • Subject to callback during emergency conditions (e.g., severe weather, evacuation, disaster, etc.).
Minimum Working Requirements
  • Must have reliable transportation, current valid Michigan driver’s license and car insurance.
  • Must be 18 years of age or older.
  • Must be fully vaccinated, or willing to be fully vaccinated against the following communicable diseases: Influenza, MMR, Varicella, Tdap/Td, Covid-19 and Hepatitis B.
