The Senior Wellness Coach will;
- Provide case management, crisis intervention and advocacy for tenants
- Collaborate with tenant in development of goals and corresponding plan
- Provide accurate, complete and timely documentation of social service records including service plans, reports and progress notes
- Assess needs and make appropriate referrals
- Participate in tenant meetings
- Develop and facilitate Substance Abuse and other Wellness groups
- Provide supervision of case management activities of Housing Generalist
- Identify and coordinate community based resources and organization for supportive Services for the Underserved
- Assist with tenant evacuation during fire drills and emergencies
- Provide recreation, money management and other hands on assistance with activities of daily living i.e. medication reminders as needed
- Report building issues to program director and superintendent
- Additional tasks as assigned by Program Director
Qualifications
- Bachelor's Degree required
- Experience with homeless services and substance abuse treatment strongly preferred
