Nurse Practitioner Role in Geriatric Care
We are partnering with a compassionate, mission-driven healthcare group dedicated to providing exceptional patient-centered care to older adults.
This is an opportunity for a dedicated and empathetic Nurse Practitioner (Family Nurse Practitioner or Adult-Gerontology Nurse Practitioner) to join a collaborative team serving residents in long-term care and skilled nursing settings.
The ideal candidate will have experience in geriatrics, long-term care, or primary care, but new graduates are also encouraged to apply.
Responsibilities:
- Evaluate, diagnose, and manage the ongoing health needs of residents in a long-term care or skilled nursing facility
- Collaborate with an interdisciplinary care team to develop and adjust individualized treatment plans
- Provide compassionate, person-centered care that improves quality of life for seniors
- Maintain clear, timely documentation and communicate effectively with facility staff and family members
Requirements:
- Active Nurse Practitioner license (APRN) FNP or AGNP
- Commitment to patient-centered care and strong communication skills
- Ability to work independently and as part of a supportive care team
