What are the responsibilities and job description for the Community Placement Program (CPP) Case Manager position at Shenandoah Valley Juvenile Center?
SVJC is hiring for a Full-Time Community Placement Program (CPP) Program Manager. The Department of Juvenile Justice (DJJ), Community Placement (CPP) Case Manager performs work related to providing services to committed residents from the DJJ. Work is performed under the general supervision of the CPP Clinician. Help residents and the parents or legal guardian to understand the effects on the resident of separation from the family and the effect of group living; Assist the resident and the family to maintain their relationships and prepare for the resident’s future care; Utilize appropriate community resources to provide services and maintain contacts with such resources; Help the resident strengthen his capacity to function productively in interpersonal relationships; Confer with staff to help them understand the resident’s needs in order to promote adjustment to group living; Work with the resident and with the family or any placing agency that may be involved in planning for the resident’s future and in preparing the resident for the return home or to another family, for independent living, or for other residential care; Attends and/or conducts staff meetings and conferences for the purpose of planning and review and problem resolution; Provide and oversee all case management services for CPP residents; Coordinate services and maintains direct contact with DJJ, Parole Officers and CPP Clinician to ensure successful execution of the individual service plan and provide follow up services; Advises and makes recommendations to the SVJC direct care staff on the residents’ needs, issues/concerns and appointments; Maintains resident record documentation to ensure compliance with the established policy; Develops and facilitates group counseling sessions to develop and foster interpersonal and social skills; Coordinate educational services with SVJC education department and education liaison.
KNOWLEDGE, SKILLS AND ABILITIES: Sound practical knowledge of case management principles, service plan development, reporting and evaluation techniques, criminal and anti-social behavior; Good decision-making and coordination and management skills; Ability to follow oral and written instructions; Good oral and written communication skills; Ability to effectively document events and situations; Ability to input data using a standard computer keyboard at a minimum rate of 30 words per minute; Understand and operate sophisticated communications and security equipment; Understand policies, procedures and services of the center; Possess mental alertness and a keen awareness of surroundings; and establish and maintain harmonious relationships with co-workers and the general public.
EDUCATION AND EXPERIENCE: A master’s degree in social work, psychology, or counseling; A baccalaureate degree in social work or psychology with documented field work experience, or A baccalaureate degree and three years of professional experience working with children.
Prefer experience working in with youth in a residential setting, or experience in the field mental health or social work.
