POSITION SUMMARY

The Community Health Representative (CHR) is responsible for providing assistance in acquiring services to program clients as directed by the Registered Nurse in coordination with other related departments and community agencies. The responsibilities include providing training in a wide variety of health-related topics on an individual, family, and community basis to clients of all ages.

 

SPECIFIC RESPONSIBILITIES 

  1. Ability to identify Native Americans in assigned service area and inform them of program services.
  2. Participate in the Outreach Program under the direction and guidance of the Registered Nurse.
  3. Provide Outreach case management and services to program clients in the service area in the following manner:
    1. Prepare and maintain a visitation schedule, with all eligible residents of the service area.
    2. Maintain a constant follow-up schedule, including clinic appointments.
    3. Maintain client files.
    4. Assist clients in identifying service needs/resources.
    5. Make appropriate in-house or out-of-agency referrals.
    6. Coordinate multi-service needs of clients.
    7. Assistant eligible clients to secure alternate resources.
    8. Provide health education, counseling, support, and services focused on general health maintenance and promotion.
  1. Actively network with a minimum of four (4) community agencies at least quarterly.
  2. Provide transportation and/or delivery of medication/food in situations of urgent need when no other means is available to the client and the consequences of not receiving such may have an immediate untoward effect.
  3. Able to understand, and be sensitive to, the culture of the Indian community to be served.
  4. Participate in Quality Improvement Program.
  5. Attend workshops and training sessions, as required.
  6. Other duties as assigned.


MINIMUM QUALIFICATIONS

  1. Must be able to read, write, follow instructions, prepare reports and demonstrate effective communication skills.
  2. Reside in the Shingle Springs Tribal Health Program Service Area.
  3. Able and willing to learn and use health care information and skills.
  4. Must possess dependable transportation.
  5. Must be able to travel for training.
  6. Must possess a valid California Driver's License and be insurable by the Tribe’s auto insurer.
  7. Will not use illegal drugs, abuse prescription drugs, or be under the influence of alcohol during working hours. Will agree to blood and/or urine level testing for drugs and/or alcohol.


DRUG SCREEN

Shingle Springs Rancheria requires the successful completion of a drug screening of all potential Tribal employees prior to being hired.  The Tribe recognizes that the State of California has legalized the use of marijuana. The Tribe is located on federally owned trust land and complies with federal laws. Therefore, marijuana use is prohibited, and all drug tests will include a screening for THC.

 

Shingle Springs Rancheria recognizes a zero-tolerance drug and alcohol policy and employees will be required to sign a Consent to Drug and/or Alcohol Testing.  Employees may be selected for alcohol/drug testing upon reasonable suspicion and upon any/all accidents.

 

BACKGROUND CHECK

Candidates for this position will be subject to criminal and character background checks and fingerprinting prior to being hired.  Verification of previous employment and education will also be conducted after the interview if an offer of employment is being considered.

Confidentiality is an absolute requirement for this position and is cause for immediate termination if not followed to the highest standard.

 

INDIAN PREFERENCE

Preference in hiring is given to the most qualified applicant.  However, when candidates for an open position are determined to be equally qualified, Native American candidates will have a preference for hiring purposes. This means that if the qualifications of the best candidates are comparable, the Native American candidate is given preference.  The following order of preference shall be adhered to when hiring: (1) Members of Shingle Springs Band of Miwok Indians members; (2) Spouses or domestic partners (certified through Tribal Court or the State of California) of members of Shingle Springs Band of Miwok Indians; 3) Parents of children of the Shingle Springs Band of Miwok Indians; 4) All other documented Native Americans; 5) All other applicants in accordance with Title 25, U.S. Code, Section 472 and 473. Applicants claiming Indian preference must submit verification of Indian eligibility.
