Responsibilities:
- Serve as a key member of the pharmacy team, collaborating with pharmacists, physicians, and other healthcare professionals to optimize patient care.
- Develop, implement, and evaluate clinical pharmacy programs and services to enhance patient outcomes.
- Provide direct patient care through medication therapy management, medication reconciliation, and patient education initiatives.
- Conduct comprehensive medication reviews and assessments to identify drug-related problems and recommend appropriate interventions.
- Participate in medication safety initiatives, adverse drug event monitoring, and quality improvement activities.
- Serve as a resource for healthcare providers regarding drug information, dosing, drug interactions, and pharmacotherapy guidelines.
- Maintain accurate documentation of clinical interventions, patient consultations, and outcomes.
- Stay current with advances in pharmacy practice, pharmacotherapy, and healthcare regulations.
- Assist with staff education and training on clinical pharmacy topics.
Qualifications:
- Current licensure as a Pharmacist in the state of West Virginia.
- Minimum of 5 years of clinical pharmacy experience in a hospital or healthcare setting preferred.
- Strong clinical knowledge and understanding of evidence-based pharmacotherapy principles.
- Excellent communication, interpersonal, and organizational skills.
- Ability to work independently as well as collaboratively within a multidisciplinary team.
- Proficiency in pharmacy software systems and electronic health records.
- Commitment to continuous learning and professional development.
Requirements
- Bachelor of Science degree in Pharmacy from an accredited college of pharmacy
- Current Pharmacist license to practice in the state required on hire
- One (1) year of previous hospital pharmacy experience
