What are the responsibilities and job description for the Support Coordinator position at Spectrum Human Services?
GENERAL STATEMENT OF DUTIES
To ensure that needed and appropriate services are provided as required by State of Michigan laws, Medicaid Manual and applicable Service Standards through the provision to direct service and contracted services. The Supports Coordinator develops and implements the Person Centered Plan with each individual assigned. Develops a therapeutic relationship with the individuals served and the significant others in his/her life in order to advocate effectively. Presents clinical information as necessary to committees, staff and other service agencies and necessary to secure services. Monitors and documents the outcome of the Person Centered Plan, revising as indicated and ensuring that all needs are being met. Identifies opportunities for improvement and actively participates in their implementation. Applies principles of CQI to all services and processes. Supports culturally competent recovery-based practices and person centered planning as a shared decision making process.
QUALIFICATIONS
EDUCATION:Bachelor’s Degree in a Human Service related field.
EXPERIENCE:Minimum of one year clinical experience with population served, meets QIDP qualifications.(Preferred)
KNOWLEDGE, SKILLS AND ABILITIES:- Demonstrated ability to communicate in a positive and support manner and to work effectively with staff, consumers and all stakeholders.
- Demonstrated experience in tracking details and managing multiple tasks and priorities.
- Knowledge of appropriate resources for eligible consumers.
REQUIREMENTS:- Licensure as Social Worker in Michigan, or qualifications to obtain. (Preferred)
- Completion of the personnel hiring requirements specified in the Conditional Job Offer at the time of hire.
- Valid Driver's License.
