What are the responsibilities and job description for the Community Liaison position at St. Barnabas?
The Community Liaison is to provide continuous support to SBH Referral Services Office. Ensuring quality and service where patients come first. Provide compassionate and exceptional service to our patients in a culturally and linguistically appropriate environment within the SBH Network.
The Community Referral Liaison works under the Supervision of the Department Director. She/he is primarily responsible for ensuring patients are cleared and scheduled for specialty and diagnostic services. The Community Referral Liaison makes best efforts to connect with the staff of the referring community physicians to maintain and update current existing data. Assist in the daily workflow of the office, including but not limited to answering a heavy volume of phone calls, data entry of referral documentation into referral system and electronic medical records. Outreach patients and providers with appointment information and follow-up with inquires.
