Status: Full-time. Exempt. 6-8 direct reports.  

Reports to: Clinical Program Manager 

Hours: Monday-Friday 8:30am-5:00pm pm plus some evenings/weekends as needed 

Salary: $75,000-$85,000/year depending on experience

Position Summary:  Reporting to the Clinical Program Manager, the Housing Program Manager provides leadership and accountability to an interdisciplinary team of professional and paraprofessional case managers and housing navigators working to support Seniors in becoming and remaining housed. Focusing on client-centered care, the Housing Program Manager ensures regular contact with Seniors enrolled in case management and housing navigation programs, documentation of services and effective follow-through, consistent contact with external partners like housing providers, and supervision of Housing Clinic and Street Outreach programs, impact measurement and program planning. 

 

Senior Homeless Services and Housing Navigation (50%) 

  • Under the direction of the Clinical Program manager, support Senior Housing Services team to ensure effective engagement with clients toward securing transitional and permanent housing, including intakes, assessments, referrals, income verification, identification and application 
  • Ensure daily documentation of client contact and progress on individual housing plans focused on securing permanent housing 
  • Conduct regular Housing Clinic onsite with support services, legal advisors, affordable housing applications and assistance in applying 
  • Document case management and housing navigation services in agency and relevant databases including Salesforce, HMIS, and others 
  • Liaison to affordable housing providers to ensure timely and appropriate information about wait lists, openings and application opportunities 
  • Support agency advocacy efforts through data collection and analysis focused on serving Seniors and those in greatest need 

  

Transitional Housing (25%) 

  • Create an environment of caring, concern and support for transitional housing clients preparing for permanent housing with public or private sector landlords 
  • Facilitate weekly house meetings and community-hosted events to provide information and improve house relationships 
  • Review and submit OHA housing applications 
  • Maintain a roster of participants, rooms, move-in dates and target exits, and a wait list 
  • Prepare, complete and upload monthly invoice and roster to SMC and OHA   
  • Create an action or repayment plan when participate cannot make monthly payment 
  • Ensure Housing Agreement is honored by participants and recommend strategies for clients in violation of Housing Agreement  
  • Conduct room inspections/wellness checks on a bimonthly basis to monitor health and safety standards   
  • Coordinate with Oakland Housing Authority, Building Inspections, Fire Safety and other agencies to ensure compliance with relevant fiscal, health and safety guidelines 
  • Ensure execution of services and client management are compliant with the guidelines established by Federal Law, ADA regulations and OHA – Moving to Work requirements 

  

 Quality Assurance, Contract Compliance and Reporting (25%) 

  • Provide input to Executive Director, Director of Programs and Impact, and Director of Finance on billing, scope of work, outcomes, and program deliverables during contract negotiations.  
  • Review language for agency compliance and alignment with best practices. 
  • Meet contract outcomes/deliverables. 
  • Collaborate with Contract and Data Team to ensure appropriate data is tracked and verified. 
  • Support Clinical Program Manager in evaluating including both program and client characteristics, i.e., evaluation outcomes based on population and service delivery 
  • Ensure regular reporting on contract deliverables and outcomes. Support monthly, quarterly and annual reports by Contract & Data team through regular data entry and recordkeeping.

  • For Medi-Cal eligible clients, this position performs Medi-Cal documentation and billing including assessment, diagnosis, referral, case plan development, consultation, brokerage, counseling, case management, crisis management.  

 

Qualifications  

  • Bachelor’s degree and 3 years’ experience serving low-income, high-need populations, especially homeless and/or formerly homeless seniors  
  • Demonstrable knowledge of federal and local housing policies, regulations and resources  
  • Experience with diverse client and staff populations, demonstrated cultural competency  
  • Excellent written and verbal communication skills  
  • Well-organized with attention to detail  
  • Strong initiative, excellent judgment and accountability  
  • Manner that models respectful relationships with program participants, staff and members of the broader community  
  • Computer competency, including Microsoft Office Suite (Word, Excel, Outlook, Powerpoint), Salesforce, HMIS, other databases

Must be able to prepare documents, research data on websites, and work on computer (with mouse or track-pad), operations up to 8 hours per day.
