Job Description
Title: Skilled Nursing RN
Exempt Status: Non-Exempt
Reports To : Unit Supervisor/ DNS/ ADNS
General Summary :
Avalon RN's are responsible for providing competent, compassionate, and high-quality care to all residents under their supervision. This includes the accurate and timely administration of medications and/or treatments, thorough and consistent health-status documentation, change reporting, responding to emergency calls, and staying in contact with the Unit Supervisor and/or DNS for assistance as needed.

Essential Job Duties :
- Supervises direct care of residents and nursing personnel on his/her shift.
- Administers direct care as necessary.
- Gives and receives shift reports.
- Rounds at least twice a shift to assure proper care is rendered in a safe and clean environment.
- Keeps physicians and families aware of changes in resident’s condition.
- Assists in developing comprehensive care plans.
Other Duties :
- Maintains the self respect, personal dignity, and physical safety of each resident.
- Maintains accurate documentation of all care rendered including Medicare required documentation to support regulations.
- Responds to resident emergency call system.
- Interfaces with other disciplines to assure continuity of care and services.
- Accompanies physicians when making rounds.
- Encourages restorative care in order to obtain maximum resident independence.
- Participates in family/resident counseling as necessary.
PHYSICAL REQUIREMENTS/WORKING CONDITIONS :
Frequent standing, stooping, bending, stretching, squatting; may be exposed to blood and body fluids which may contain HIV and/or HBV; must be able to transport residents via wheelchair, geri-chair or shower chair, pushing up to 150 lbs. unassisted. Must be able to transfer, lift, turn and position a resident weighing up to 125 lbs. unassisted, or with the assistance of one for a resident weighing over 125 lbs. Must be able to stoop, bend, stretch, squat, stand, and walk for up to 90% of the workday. May be subjected to offensive odors and combative behavior. Must be able to reach up to a level of six feet; must be able to push a dietary cart up to 40 lbs unassisted. Must be able to lift and carry up to 20 lbs. frequently and 50 lbs. occasionally.
