What are the responsibilities and job description for the Med Specialty Triage - RN position at Talent Consultants?
We are seeking a skilled Registered Nurse (RN) for the Medical Specialty Triage Area. You will play a critical role in the initial clinical evaluation, determining acuity levels, and facilitating timely care coordination across 12 specialty clinics (including Cardiology, Diabetic Educator, Endocrine, Hematology/Oncology, Rheumatology, Medical Injections, and Pulmonary). You will work closely with physicians and other healthcare professionals to support patient-centered care, focusing on wellness and disease management. Your expertise will be essential in assessing patient needs and coordinating healthcare services to ensure optimal patient outcomes. You will also be expected to respond to urgent clinical situations, including escalating care and escorting Veterans to the Emergency Department when medically necessary. You will have the opportunity to float to the various clinics as desired and as competencies are met. Ideal candidates are confident in clinical decision-making, possess strong assessment skills, and capable of navigating multiple service lines while maintaining a focus on compassionate care.
Responsibilities:
- Patient Assessment and Care: Conduct thorough initial patient assessments, including health history, physical examinations, and symptom evaluation. Monitor and manage patient conditions, addressing acute and chronic illnesses, and collaborating with physicians on treatment plans.
- Care Coordination: Coordinate patient care activities, including referrals to specialists, diagnostic tests, and follow-up appointments. Ensure seamless transitions between healthcare settings and maintain communication with interdisciplinary team members.
- Documentation and Record Keeping: Maintain accurate and detailed patient records, including medical histories, symptoms, treatments, and outcomes. Adhere to regulatory requirements and organizational policies for documentation and reporting.
- Patient Advocacy: Advocate for patients' rights and preferences, ensuring their voices are heard in healthcare decision-making processes. Serve as a liaison between patients, families, and healthcare providers to facilitate open communication and shared decision-making.
- Quality Improvement: Participate in quality improvement initiatives to enhance patient care outcomes and operational efficiencies. Contribute to evidence-based practice guidelines and clinical protocols.
- Emergency Response: Assess and respond to medical emergencies within the scope of practice, providing immediate care and initiating emergency protocols as needed.
Mandatory Requirements:
- Licensure: Current, unrestricted RN license from any state in the United States.
- Certification: Current Cardiopulmonary Resuscitation (CPR)/Basic Life Support (BLS) certificate accredited by the American Heart Association.
- Experience: Minimum of three (3) years prior experience in a direct care setting setting (e.g., Primary Care, Medical-Surgical role, etc.).
- Education: Graduate of a school of professional nursing approved by the appropriate state-accrediting agency and accredited by ACEN or CCNE.
