Responsibilities:
- Supervise and lead home health services, ensuring safe and therapeutically effective care for patients and families.
- Develop, implement, and evaluate departmental objectives, policies, and procedures in line with Medicare, Medicaid, and accrediting bodies.
- Hire, mentor, evaluate, and, if necessary, terminate staff; oversee orientation and ongoing staff development.
- Coordinate patient care—ensuring ongoing assessment, interdisciplinary planning, and physician-approved plans of care.
- Monitor quality and performance improvement through ongoing QAPI oversight, including data analysis and action planning.
- Manage confidentiality and patient safety, investigating and resolving concerns related to care delivery and workforce wellness.
- Plan and facilitate staff meetings, case conferences, and community education initiatives while maintaining thorough documentation.
- Collaborate with the leadership team to identify budget requirements, ensure appropriate resource allocation, and optimize equipment use.
Must-Have Skills:
- Current Registered Nurse (RN) license in the state of Missouri.
- Three to five years of experience in health care or home care settings.
- Ability to supervise, guide, and develop performance within clinical teams.
- Solid understanding of community health principles and care standards.
- Proven independent judgment, decision-making ability, and initiative in a leadership context.
Nice-to-Have Skills:
- Two years of direct supervisory experience in home health or health care management roles.
- Experience designing and maintaining quality assurance and performance improvement (QAPI) programs.
- Knowledge of relevant Medicare, Medicaid, and industry regulations and accreditation requirements.
- Strong capability to foster teamwork, cooperation, and the advancement of organizational goals.
- Demonstrated skill in staff training, community engagement, and process improvement initiatives.
