Location: Madison/Dane County
Position Summary
The Clinical Case Manager is responsible for case management services provided to clients at the Dane County Care Center and is expected to provide clients with clinical and supportive social services with the goal of supporting clients find ways to manage AODA, mental health, family, personal, health, and other functional problems. The Clinical Case Manager works with clients to develop and implement treatment planning and services. The Clinical Case Manager will be responsible for individual, group, and other appropriate clinical services within the need of the program and the scope of their training.

Essential Functions
- Interview clients to assess their acute needs and challenges.
- Review client records and confer with other professionals to determine appropriate referrals to community services.
- Maintain knowledge of community services and resources related to mental health and substance use disorder treatment in Dane County
- Collaborate with community providers to plan or coordinate services that meet client needs.
- Work directly with clients with mental health diagnoses and assist them in understanding their mental health and substance use issues.
- Assist clients in obtaining financial resources and housing.
- Assist clients in coordinating care, such as setting up appointments, arranging for transportation to appointments, and providing support.
- Refer clients to community resources to assist with their recovery from substance use disorder.
- Monitor, evaluate, and document client progress with respect to client treatment plan goals.
- Modify treatment plans according to changes in the client's status.
- Support Behavioral Health Technicians in their role as direct care staff to provide quality of care and services.
- Enter data for reporting purposes and generate reports as required.
- Increase knowledge relating to the client population by reviewing current literature, conducting research, or attending seminars, training workshops, or classes.

- Master’s degree in social work
- Two or more years of related experience required.
- A Valid Driver's License is required
