Seeking a Case Manager to join our interdisciplinary clinical team working with individuals diagnosed with a Serious Mental Illness (SMI) or General Mental Health diagnosis under Court Ordered Treatment at our Desert Cove Clinic.

Duties may include :
- Assists clients with identifying treatment and recovery goals and provides supportive services to help clients reach their goals
- Completes assessments, crisis plans and individualized service plans
Works collaboratively with the clinical team to engage, educate, communicate and coordinate care with member, their family, behavioral health, medical and dental providers, community resources and others in ensuring that all services prescribed in the individualized service plan are implemented
Conducts outreach and follow-up of services including, but not limited to, crisis interaction, missed appointments, and hospital discharges to ensure adequate resources are available and in place.Performs all case management functions associated with caseload including completing the assessment and service planning processes; including identifying the need for further or specialty evaluationsPursues best practice outcomes for person with mental illness including continuing education, employment, independent housing and community tenureBenefits & Wellness :

GED / High school diploma with a minimum of 1.5 years of behavioral health experience, orAn associates degree in the field of medical or behavioral, orA bachelor's degree (not in the field of behavioral health) and 1 year of behavioral health experience, orA bachelor's degree in the field of behavioral, orA master's degree or higher in the field of behavioral healthComputer Skills : Microsoft Word, Excel, Outlook, electronic medical record, NextGen preferredValid AZ Driver License. Must have valid Arizona driver's license, be 21 years of age with minimum 4 years driving experience, and meet requirements of Terros Health's driving policyMust have a valid Arizona Fingerprint Clearance card (Level 1) or apply for an Arizona Fingerprint Clearance card within 7 working days of employmentInterpersonal skills which allow for professional and positive relationships with co-workers, members, families, and other community membersGood communication skills, evidenced by a non-judgmental approach, effective listening and positive interactionsBasic understanding of the DSM-V preferredCertified Case Manager strongly preferredMust pass background check, TB test and other pre-employment screening.
