Description
The Mental Health Clinician collaborates with a multi-disciplinary team to provide services consistent with the program model; and participates in daily team meetings to review and plan client services. The Mental Health Clinician provides the expected number of services to clients each month 80% of which are in the community; completes required documentation in a timely and high-quality manner; works collaboratively with clients, their families, and community partners to assist clients in achieving their treatment and recovery goals.

Essential Position Functions:
- Work with clients, families, and/or significant others to develop a person-centered treatment plan.
- Lead team efforts in screening and assessing clients for the treatment of co-occurring substance use disorders, including involvement in an interdisciplinary assessment.
- Advocate and liaise for clients within the criminal justice and shelter systems.
- Assist clients in securing medical care, entitlements, and other community supports that promote integrated physical and mental health, safety, well-being, and recovery.
- Provide or connect clients with employment counseling, vocational rehabilitation, housing applications, placement, and life skills training.
- Provide team training and individual counseling that includes principles of Integrated Dual Disorder Treatment, as appropriate.
- Complete progress notes and assessments in the electronic health record in a timely manner.
- Work with external stakeholders, such as hospitals, shelters, jails, and primary care, to achieve positive client outcomes.
- Provide services in a strengths-based, recovery-oriented, trauma-informed, and culturally sensitive manner.
- Perform any other duties as assigned.
Qualifications
Qualifications, Skills, and Knowledge:
- Must have or be eligible to have NY State ID (non-driver or driver's license).
- Education: MSW, LMSW, LMHC, LMHC-LP
- Licensed and Unlicensed Mental Health Clinicians are welcome to apply!
- Experience: At Least two years of experience working with adults who have been diagnosed with a serious mental health diagnosis.
- Knowledge and experience of electronic health records (specifically AWARDS by Footholds) preferred.
- Spanish speaking is highly desirable.
- Experience working with homeless and/or precariously housed populations is preferred but not required.
- Schedule: Monday – Friday 9:00 AM – 5:00 PM
