The Cedars is looking for an LBSW to join our team!Position Overview:The primary responsibility of the Social Worker is to plan, organize, implement, evaluate and direct the Social Service Department in compliance with Federal, State and local regulations/standards, and as directed by the Executive Director, to assure the emotional, physical, and spiritual needs of residents are maintained. The Social Worker is responsible/accountable for the overall social services program of the facility Essential Job Functions:Provide overall direction of Social Services Department for Healthcare and Assisted Living.Work with Senior Living Advisors Team (SLAT) to assess new admissions to health careOversee the quarterly Comfort Matters training for all current newly hired staffMaintain social history on each residentMaintain budget and suppliesIdentify medically related social/emotional/spiritual needs of residentsRemain current with State, Federal and local regulations and standardsAttend Care Plan meetings, complete MDS on time, and assist with P.O.C.Attend meetings as assigned, such as Medicare, management, etc.Review resident rights with residents and staff each yearOversee the admission and discharge process and maintain records per facility policyComplete resident assessments as requiredDocument interactions with residentsOrganize and attend monthly Resident Council meeting for HealthcareInvestigate and maintain log of resident complaints and grievancesProvide resident/responsible person with resident rights, DPOA, loving will, DNR forms as neededAssist with investigation of all complaints of abuse, neglect, and/ or exploitationMaintain monthly social progress notesAssist in scheduling appointments for mental health servicesAttend continuing education to keep license currentAssist as needed to have area-wide social service meetingsAssist as needed with activitiesComplete new admit paper work with resident and/or familiesComplete PASSARS upon admit and on an as needed basis Qualifications and Experience:Must be at least eighteen [18] years of ageMust have a Bachelor’s Degree in Social Work accredited by the Council on Social WorkMust be licensed by the Kansas Behavioral Sciences Regulatory BoardPrefer one-year experience in Long-term Care settingMust be able to deal tactfully with staff, residents’ families and general publicMust be able to adapt to a flexible work schedule Physical Requirements:Light Work – Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently. Requires walking or standing to a significant degree; or requires sitting most of the time but entails pushing and/or pulling of arm or leg controls.
