Job Opportunity: Intensive Case Manager – Next Step Program
The Chamber Foundation, Inc. is seeking a dedicated and compassionate Intensive Case Manager for our Next Step Program. This role involves working directly with program participants to provide comprehensive job-seeking, job retention, and support services in alignment with the DCFS Workforce Connections Contract. As a key member of our team, you’ll help individuals achieve long-term career success while fostering a positive relationship with employers and community organizations.
Key Responsibilities:
- Program Promotion & Employer Outreach: Market the Workforce Connections Program to community organizations and potential employers, showcasing the program's benefits to local social service and employment networks.
- Participant Enrollment & Support: Guide new participants through the enrollment process, ensuring all documentation is completed accurately, including participant profiles, confidentiality agreements, job-readiness interviews, and SMART goals.
- Workshops & Training: Develop and implement workshops on job readiness, soft skills development, budgeting, conflict resolution, and more. Collaborate with local training providers such as DC BOCES and DCC to offer additional learning opportunities.
- Ongoing Communication & Support: Maintain regular contact with participants, providing weekly check-ins for up to six months, assisting with any challenges, and ensuring their continued success in the workforce.
- Employer Relations: Work closely with employers to ensure the success of participants in their jobs. This includes meeting with job supervisors, monitoring job performance, and addressing any issues that arise. Maintain frequent communication with employers, tracking all interactions in the Civicore database.
- Confidentiality & Case Documentation: Uphold confidentiality in all communications, document case progress, and submit monthly reports on participant success to DCFS.
- Home & Community Visits: Conduct home visits and attend community-based appointments to provide additional support as needed.
- Collaboration & Reporting: Work collaboratively with Chamber staff, DCFS representatives, and external agencies to ensure smooth program implementation and successful outcomes for participants.
Qualifications:
- Education: Bachelor's degree in Social Work, Psychology, Counseling, or a related field.
- Experience: Experience working with individuals facing mental health challenges, criminal justice involvement, and disabilities. Knowledge of case management principles and employment services is preferred.
- Skills: Strong communication, interpersonal, and problem-solving skills, with the ability to work effectively with diverse populations in a multidisciplinary team setting.
- Additional: Bi-lingual skills are a plus. A clean driver’s license and reliable transportation are required for travel throughout the region.
