We are seeking a compassionate and experienced Lead Care Coordinator to join our team dedicated to providing comprehensive care for the homeless population suffering medical crises. As the Lead Care Coordinator, you will oversee all care coordination / case management duties, ensuring the provision of high-quality support services to our clients. This role requires strong leadership skills, a deep understanding of social work principles, and the ability to collaborate effectively with multidisciplinary teams.
RESPONSIBILITIES
Team Leadership : Supervise a team of care coordinators, floor managers, transportation staff, and resident advocates, ensuring alignment with best practices and ethical standards in social work. Support our volunteer program to achieve department goals.
- Care Coordination / Case Management : Responsible for the intake of new residents as well as their needs assessment, development, implementation, and review of individualized case / goal plans for clients, with detailed goals and objectives, ensuring they receive comprehensive support tailored to their needs. Competency in both strengths-based case management, housing first principles, as well as medical case management to ensure medical respite care plans are managed.
- Client Advocacy : Actively advocate for residents' rights fostering positive interactions with external agencies and community resources.
- Staff Training and Development : Provide ongoing training, coaching, and professional development opportunities to case management staff, fostering a supportive and empowering work environment.
- Collaboration and Coordination : Collaborate closely with medical staff, mental health professionals, substance abuse counselors, and other service providers to ensure integrated and coordinated care for clients.
- Documentation and Reporting : Maintain accurate and up-to-date client records, progress notes, and reports, ensuring compliance with regulatory requirements and funding guidelines in The INN Betweens platforms (UHMIS / EMR).
- Crisis Management : Respond to behavioral emergencies and provide on-call support for night and weekend staff.
- Quality Improvement : Participate in program evaluations and implement changes to enhance service delivery.
- Community Engagement : Represent The INN Between in external collaborations, ensuring alignment with the organizations mission and goals.
- Compliance : Ensure adherence to all relevant regulations, policies, and procedures governing social service provision and confidentiality.
