What are the responsibilities and job description for the Case Manager - DDA Services position at The League for People With Disabilities?

The Case Manager utilizes a person-centered approach to write Service Implementation Plans (SIP) by establishing meaningful, measurable, and individualized goals for participants receiving Meaningful Day, Employment, Personal Supports, and Behavioral Support Services. The Case Manager will also monitor progress towards goals by completing quarterly goal reviews and maintaining ongoing communication with the DDA Coordination of Community Service (CCS), Direct Service Professionals (DSP), and all other team members.Essential Duties and Responsibilities : Coordinate with DDA CCS’s to schedule initial, annual, and revised Person-Centered Plan (PCP) meetings with all relevant team members.Complete 90-Day Reviews for each participant by reviewing progress towards goals and making recommendations as needed (augmenting specific measures, implementation strategies, etc.).Maintain up-to-date participant files and databases.Provide quality case management services to meet participant needs as they arise, including providing community resources.Conduct monthly communication with participants and caregivers regarding participant progress and needs.Serve as the primary Incident Report writer for the department, adhering to reporting timelines.Attend transition & resource fairs to market League programs and services.Additional Duties and Responsibilities : Assist the Intake Coordinator in reviewing and processing new applications for program participants.Additional responsibilities as assigned by management.Demonstrate the 5 Core Values of The League (Accountability, Participant Focus, Quality, Independence, and Mutual Respect).Operationalize the mission of The League for People with Disabilities by creating a supportive environment for participants and staff.Qualifications Requirements : Bachelor’s Degree in a related field preferred.HS Diploma with at least 3 years of successful experience in case management and / or working with individuals with disabilities required.Must have knowledge of, or the ability to gain understanding of, DDA and DORS regulations quickly.Must possess basic computer skills, including proficiency in Microsoft Word, Excel, and Outlook.Must have excellent written and oral communication skills.Must have exceptional organizational skills and the ability to work with autonomy.#J-18808-Ljbffr
