LPN Unit Manager

$70,000 Annual Salary with Full 1199 SEIU Union Benefits and $3,000 Sign-ON Bonus
The Paramount at Somers Rehabilitation and Nursing Center seeking experienced manager.

- Direct Management of 50 Bed SNF unit and unit staff
- Assist with admission, transfer and discharging of residents
- Correspond with family members
- Educate unit staff
- Responsible for resident’s living environment
- Participate in Care Plan development
- Educate residents and their family members regarding treatment, rehabilitation, change of status, etc.
- Adhering to infection control policies and procedures as well as all other facility policies and procedures
- Consult regularly with physician regarding resident’s plan of care
- Manage staff assignments
- Adhere to DOH standards of practice
- Work directly with the interdisciplinary team
- Member of the Care Plan team
- Any other tasks as designated by the manager or Nursing Administration.
Requirements/Qualifications
- Must possess a nursing degree from an accredited college or university or be a graduate of an approved LPN program
- Valid LPN license
- Internal candidates must be in good standing
- 2 years of LTC LPN experience preferred; post-acute experience a plus
- Excellent communicator, positive attitude, team player
