MINIMUM QUALIFICATIONS:
- Must have excellent communication and interpersonal skills.
- Must be fluent in the English language: reading, writing, speaking.
- Must have basic skills in Microsoft Office Outlook, Word and basic computer skills.
- Must be able to successfully complete the background screening process to include, but not limited to, a drug test and finger printing.
PREFERRED QUALIFICATIONS:
- Bachelor’s degree in Social Work (BSW).
- Two (2) or more year's experience in a health care setting.
- One (1) year experience in a health care setting as a social worker.
- Experience working in a Nursing Home.
- High school Diploma or GED.
POSITION DESCRIPTION:
Serves on Care Plan team with the responsibility of insuring that the resident treatment objectives are consistent with his/her overall treatment process.
Maintain progress notes on each resident as needed.
Provides social and behavioral expertise in the development of treatment plans and management of social services for residents and/or families with psychosocial needs resulting from complex medical problems.
Consults with doctor, nurses and other professionals regarding emotional and behavioral problems encountered in the treatment process.
Administers evaluation assessment, through interview, to determine individual therapeutic program needs and interests.
Use such findings to develop, organize and implement innovative therapeutic programs to meet the mental health needs of the individual resident.
Interviews residents and relatives regarding admission procedures and release plans.
