The Waters of Smyrna is now hiring for full time Hospital Liaison
Position Summary:
The Hospital Liaison has an essential role in ensuring a seamless transition for all admissions from acute or post-acute
care. Your collaborative engagement is significant to coordinate admissions and communication to maintain continuity,
and ensure the best care, our communities have come to expect. As a Hospital Liaison, you will generate and manage
referrals and admissions from hospitals. You will take lead developing relationships with hospital discharge planners,
and developing initiatives that align with market to grow business and identify new opportunities. Hospital Liaison works
very closely with our Admission Directors and leadership, to drive daily admissions. Time management, and multitasking will be necessary for market growth.
ESSENTIAL JOB FUNCTIONS
A. Role Responsibilities- Administrative:
- Stationed daily at primary account to maximize market engagement, and referral review efforts.
- Review and update CRM Dashboard for accurate reporting via Forecast and other communication platforms.
- Respond to all incoming referrals on Careport and/or other referral platforms timely, daily, and throughout
- Provide definitive answers for all incoming referrals within an hour of receiving and/or by end of day, if
- Communicate/Collaborate with Business Development and Operations leadership to expedite any delays,
- Collaborate with admissions on communication with incoming referral, and admissions on CarePort, and
- Timely PCC data input including patient demographics, resident representative, POA, and Guardian
- Weekly Report
- Participate in Quarterly Business Development meeting, aligning initiatives with your market knowledge, to
- Complete daily Forecast for Team review via Census call, Morning meetings and/or standdown.
- Overseeing Guest Relations and Concierge initiatives including patient rounding, assisting with testimonials,
- Other tasks as assigned.
- Aware of, and adhere to, Resident Bill of Rights and Confidentiality of Resident Information.
- Interact with residents, family members, co-workers, clinical and ancillary staff in a respectful and

- Referral On-site, providing collateral, business cards, and details for placement.
- Schedule market development activities including, but not limited to CEU’s, Hospital info sessions.
- Networking with Home Health, Supportive Living, Assisted Living, and other communities in market to
- Collaborate Business Development and Administration on Physician Development and communication.
- Collaborate with Business Development and Administration for Service line development, and training.
- Conduct facility tours and daily patient rounding.
- Role Responsibilities – Safety and Infection Control:
- Follow all safety, and OSHA precautions in the performance of all duties.
- Report all accidents/incidents, safety violations, hazardous conditions, or defective equipment according to

- Maintain the confidentiality of all resident care information and complies with all HIPAA standards.
- Ensure that all residents are treated fairly, with kindness, dignity, and respect. Any deviation observed,
immediate threat.
- Uphold compliance with all rules regarding Residents’ Rights.
- Immediately report all allegations of misappropriation of resident property according to facility policy
