Looking for Care Coordinator-RN in Fort Defiance, AZ,
13 Weeks Contract,
Shift: M-F, 8-5/730-430, no holidays no weekends. Maximum of 2 Days Call off (Unapproved RTO) during contract.
36 Guaranteed hours per week,
Title: Care Coordinator (case manager) nurse positions within our Primary Care clinic:
Requirement:
Shift: Weekdays Monday through Fridays from 8a to 5p, with One (1) hour lunch, no weekends or holidays. Collaborate with support nursing staff and with providers for patient care.
Job Type: Contract
Pay: $60.00 - $65.00 per hour
Expected hours: 36 per week
