What are the responsibilities and job description for the REGISTERED NURSE position at Town of Nantucket?

The Town’s nursing facility, Our Island Home, seeks a Registered Nurse (RN) to provide and oversee direct patient care to the residents of the Islands only nursing home.

 

Our Island Home is a 45-bed skilled nursing facility dually licensed in Medicaid and Medicare and owned by the Town of Nantucket. The Resident population is diverse in not only heritage but origin of location.

You must be currently licensed in MA and be in good physical and mental health. Completion of an accredited nurse licensure program is required; you must comply with the regulations of the MA Department of Public Health. Prior work experience in long-term care preferred, but not required.

Hourly rate $49.21 (new hires) - $62.34 (employees hired prior to July 1, 2012 with 20 years of service with the Town of Nantucket)

CHARGE NURSE:

Job Definition

Works 7-3 or 3-11 or 11-7 to provide assistance in planning, coordinating, and implementing patient care in an assigned area.

Supervision

The Charge Nurse is supervised by the Director of Nursing.


MEDICATION NURSE:

Supervision:

The Medication Nurse is supervised by the Director of Nursing and works with the Nurse in Charge.


TREATMENT NURSE:

Supervision:

The treatment Nurse is supervised by the Director of Nursing and works with the Nurse in Charge.

CHARGE NURSE:

  1. Interprets existing policies, procedures and performance standards to personnel, residents, families, physicians, community agency, consultants, and ancillary service providers.
  2. Participates in the facility educational programs, including but not limited to orientations and in-services.
  3. The Charge Nurse is responsible for the performance of total nursing care of the residents in our OIH during his/her tour of duty with the assistance of auxiliary nursing personnel and with the supervision of the Director of Nursing. 
  4. Receives report when coming on duty and takes follow-up action as necessary.
  5. Delegates daily duties, scheduling employee for proper Resident care.  Supervises Resident care; instructs and supervises nursing personnel.   
  6. Prepares, distributes and charts medications and treatments at specified times.
  7. Conducts resident rounds during assigned duty and identifies problems, concerns, or situations that would require intervention or corrective action and reports them to Director of Nursing.
  8. Contact physicians when necessary; takes verbal or telephone orders and/or transfers orders from doctors’ order book to Kardex, medication sheet and medication cards.  Accompanies physicians on rounds to assist, answer questions, receive orders and instructions and make notations on resident care requirements.
  9. Consults with Director of Nursing on specific nursing problems and/or interrelating department situations.
  10. Assists Director of Nursing in preparation of employee performance evaluations and makes appropriate recommendations.
  11. Contacts Resident, next of kin, or sponsor and clergy when warranted concerning changes in residents’ condition, transfer or admission.
  12. With the assistance or in the absence of the Director of Nursing, attends to Resident care plan meetings; reviews with Social Worker, Activities Director, Nurses’ Aides, Physiotherapist and pharmacist. 
  13. Assist in completing and/or updating Resident summaries and Resident Care Plans.
  14. Keeps record of routine heights, weights, and blood pressures of Residents.
  15. Admits Residents, transfers Residents, and prepares Resident care referral forms. 
  16. Prepares and maintains residents’ clinical records, including medical and nursing treatments and related services provided by the nursing staff.
  17. Complete daily reports on staff hours worked, care and condition of Residents, etc.  Investigates and adjusts complaints or refers same to Director of Nursing.
  18. Reviews medications of Residents monthly and is alert at all times for drug reactions and reports same to proper physician and pharmacy.
  19. Performs duties relative to the ordering of medications, preparation and distribution of medications, and the charting of medications; is responsible for the maintenance of Emergency Drug Box and for the disposal of unused medications and unused narcotics; maintains Narcotic Book, i.e., narcotic count.
  20. Works with team members and other staff members in a helpful, respectful, and courteous manner; must be able to establish and maintain cooperative working relationships.
  21. Maintains appropriate attendance records. Arrives on time for work and is ready to perform assigned tasks.
  22. Meets facility hygiene and dress code standards of appearance.
  23. Follows facility conduct standards and policies and procedures performing all work alcohol and drug free.
  24. Meets requirements for in-service education and meeting attendance per facility policy and state and federal requirements.
  25. Maintains all required licenses, certifications, qualifications and associated standards.
  26. Performs related duties as may be required.


MEDICATION NURSE:

  1. Administers medications safely according to policy and procedure while adhering to State and Federal regulations, within his/her scope of practice, and in accordance to Physicians’ orders.
  2. Documents appropriately in the medication records, drug order book, flow sheets, and narcotic record log.
  3. Maintains medications in the medication room and the medication cart.
  4. Is responsible for the security of medications.
  5. Is responsible for the cleaning and stocking of the medication areas.
  6. Assures that the medication refrigerator is the appropriate temperature for storing vaccines and biologicals.
  7. Assures that the medication refrigerator is clean, stocked, and free from food.
  8. Is responsible for replacing sharps containers as needed.
  9. Orders new medication from the pharmacy.
  10. Assists charge nurse in her responsibilities.
  11. Reorders e-kits of all types when needed.
  12. Assures that all medication orders are received and logged according to policy and procedure. Follows up with the pharmacy if discrepancies are found.
  13. Arrives on time for work and is ready to perform assigned tasks.
  14. Meets facility hygiene and dress code standards of appearance.
  15. Follows infection control policy and procedure while performing duties.
  16. Follows facility conduct standards and policies and procedures performing all work alcohol and drug free.
  17. Meets requirements for in-service education and meeting attendance per facility policy and state and federal requirements.
  18. Maintains all required licenses, certifications, qualifications and associated standards.
  19. Performs such other duties as may be required in coordination of nursing services.


TREATMENT NURSE:

  1. Provides assistance with treatments, medications, admissions and other nursing care when necessary.
  2. Keeps record of routine heights, weights and blood pressures of residents.
  3. Orders medications weekly from pharmacist.
  4. Participates in monthly pacemaker checks.
  5. Is responsible for sterilization of instruments, thermometers and other equipment.
  6. Assists charge nurse in her responsibilities.
  7. Assists consulting M.D. staff with procedures.
  8. Weekly Quality Control Accutest check and routine cleaning of monitor.
  9. Is responsible for assessments as needed of skin tears, rashes, etc.
  10. Quarterly assessments of restraints, bladder and pressure sore risks.
  11. Set up bladder training programs.
  12. Bowel disimpaction as needed.
  13. Cleaning and storage of equipment such as hydroculator, suction and oxygen equipment.
  14. Maintains appropriate attendance records. Arrives on time for work and is ready to perform assigned tasks.
  15. Meets facility hygiene and dress code standards of appearance.
  16. Follows facility conduct standards and policies and procedures performing all work alcohol and drug free.
  17. Meets requirements for in-service education and meeting attendance per facility policy and state and federal requirements.
  18. Maintains all required licenses, certifications, qualifications and associated standards.
  19. Performs such other duties as may be required in coordination of nursing services.

CHARGE NURSE:

Education and Experience:

The Charge Nurse shall be currently licensed in Massachusetts and must be in good physical and mental health. Completion of an accredited nurse licensure program or if licensed by waiver, she/he must comply with regulations of D.P.H. Completion of an accredited nurse licensure program or license that complies with all appropriate D.P.H. regulations. Prior work experience in the healthcare field is preferred.

Physical Requirements: 

Must be able to lift and/or move objects no less than 50 pounds. More intensive physical effort may be necessary to assist with residents when needed. Requires frequent heavy lifting. Involves standing, sitting and/or walking for the greater portion of the workday. Mental and physical fitness is required for completing the tasks involved in the position.
