**Shift: 2:00 PM - 10:00 PM Experience:• 24 months of role experience is required from any number of months.
• 12 months of Long Term Acute Care Unit (LPN) experience is required from any number of months.
• Experience with Epic is required from last 12 months.
Requirements:
• Candidates must have a New York license (required for submission).
• This role may require floating to additional units and locations
• Local & travel allowed.
• COVID vaccination required for submission. Declinations not accepted.
• COVID booster required after submission. Declinations not accepted.
• Flu vaccination required after submission. Medical declinations accepted.
Pre-employment modules may be required for this role. Please upload any certifications or health documents you have to your profile to expedite your on-boarding process.
Additional Details:
Assignment for up to 20 weeks.
Every other weekend, no call, no self-scheduling but may pick up shifts.
LTC experience. LPN is required to cover CNA duties as needed.
Bi-weekly Orientations on Monday.
No block schedules for any reason.
This facility requires ALL work history information (bed count, EMR, facility type, etc.) to be complete in order to submit. Please ensure all employment gaps over 30 days are explained.
Public Transportation is NOT accessible in this area, please ensure you have reliable transportation, personal vehicle or rental vehicle are strongly recommended. Vehicles should be equipped with all season or snow tires during the months of November – April
