Twin County Regional Hospital is a 141-bed facility that provides acute care, ambulatory care, and emergency services. Our hospital offers a wide range of healthcare services, including emergency medicine, heart care, maternity care, sleep medicine, and surgical services.
Care Coordination
As an RN Case Manager, you will be responsible for the overall management of care coordination for observation and inpatients in collaboration with the care team. This includes ensuring the appropriateness of care and resource utilization, as well as continued stay reviews.
Education and Support
You will provide education to patients as an adjunct to the care team, particularly in regards to chronic disease management. We also offer opportunities for further education and training to support your career growth.
