Position Title: Care Coordinator
Program: A New Start
Pay Range: $22-$27 per hour
Classification: Non-exempt, Full Time
Summary and Range of Authority: A New Start is an outpatient MAT (Medically Assisted Treatment) program for 12-25 years old to access MAT services. Under the direction of the Director, the Care Coordinator supports the program by working with this population in screening referred clients for appropriateness, provide client education, refer ineligible clients to other providers for services, and link clients to other community resources
Essential Duties and Responsibilities:
· Serves as Care Coordinator for clients within the program.
· Provide education and supportive services to clients and families
· Performs individual care coordination services as needed
· Provide linkages and referral services
· Process referrals from various referral sources such as criminal justice system, Child Welfare Services, recovery residences, other various providers and community agencies and individuals.
· Completes documentation for any clients who utilize care coordination services.
· Functions as a member of a multi-disciplinary team and participates in case discussions around clients.
· Provide culturally competent services. Work well with TAY, immigrants, refugees, and other culturally diverse clients
· Prepares medical records of all services rendered including progress notes, treatment plans, outcome measures, etc.
· Monitors clients for any high-risk factors on a regular basis
· Monitors clients for any primary care health needs which require medical attention
· Works collaboratively with and receives support from other members of the team to manage clients that are at high risk or in immediate crisis.
· Informs supervisors immediately if a client is in crisis
· Participates in staff meetings, training and quality assurance activities as needed
· Conducts outreach at community health fairs or ethnic community fairs as needed
· Maintains productivity requirements
· Other duties as assigned
Requirements
Required Education, Certifications/Licenses and Language/Cultural Skills:
· Minimum Bachelor’s degree in social work, psychology, counseling or human behavior from accredited college or university with major work in clinical therapy.
· Certified by one of the DHCS-approved certifying organizations (OR)
· Registered by one of DHCS approved certifying organizations
From the date of registry, counselors have five (5) years to become certified. If counselor fails to become certified after being registered for 5 years, the counselor will not be permitted to provide counseling services to clients per State regulation and therefore will no longer be employed.
Minimum Experience and Qualifications Required:
· Minimum of 4 years of clinical experience in a substance use/MAT setting
· Familiarity with community resources and the delivery of case management services
· Ability to establish and maintain diagnostic and psychotherapeutic relationships with clear understanding of ethical and professional standards
· Ability to maintain strict confidentiality with regard to client information and other sensitive or proprietary information
· Ability to communicate substance abuse/MAT principles to clients, families and providers
· Ability to work independently and also collaborate as a member of an interdisciplinary team
· Excellent written and verbal communication skills
· Ability to prepare written reports accurately and records of services rendered in a timely fashion
· Will not be debarred or excluded from participation in Federal programs by the General Services Administration and/or the Department of Health and Human Services of the Inspector General
· Must pass background check (Live Scan Fingerprint).
· Must be able to drive (when needed) for business purposes to other program locations in San Diego County. If employee drives his/her personal automobile for UPAC business, employee must have valid California driver’s license and comprehensive automobile insurance coverage, as required by law.
Additional Preferred Education or Qualifications:
· Prior knowledge and experience of using an EHR system.
· Bilingual/bicultural
Physical Demands and Work Environment:
The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Spends approximately 70% of work time sitting and meeting with others or working at a desk and/or computer. Spends approximately 30% of work time standing or walking within the work area or outside the office at meetings and events, also bends, twists, stoops and reaches. Ability to communicate in writing and verbally. Regularly required to sit and talk or hear. Frequently is required to use hands to touch, handle or feel and reach with hands and arms. Occasionally lifts and/or moves up to 25 pounds. Working conditions are normal for an office environment and event venues. The noise level is usually moderate.
