Description
UCFS is currently seeking a full-time LPN to provide general nursing care to patients in the ambulatory care setting.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Assists with physical exams, completes required screenings and laboratory testing
- Assist in office procedures including wound care
- Administers treatments/immunizations/medications to clients as ordered by the physician
- Provides client services; educate client/parent as needed; assists the client with prescription needs/refills coordinates services with/for the client; acts as a client advocate to pursue financial assistance, initiates referrals to community-based organizations as needed (Home Care, Healthy Start, Dental, and Behavioral Health)
- Responds to calls from patients, pharmacies, home care agencies and family members; collaborates with school nurses, WIC colleagues, DCF and other medical departments as needed. Completes paperwork needed by external organizations
- Telephone and face-to-face triage to assess changes in condition using established protocols
- Completes and maintains all required documentation
- Ensures a clean, organized work environment
- Treats co-workers, patients, and visitors respectfully, courteously and professionally, addressing conflict and disagreement in a positive and respectful manner
- Reports communicable diseases to the local state department of public health
- Works collaboratively with providers to ensure timely completion of paperwork including FMLA, disability, etc
Requirements
Qualifications –
- High School degree and LPN training course.
- One to three years of experience in a healthcare setting. Prior experience in ambulatory care strongly preferred
- Licensed Practical Nurse, Licensed to practice in the State of Connecticut
- CPR certified
