Responsible for the coordination of patient and family discharge needs which involves collaboration with various members of the health care team across the continuum of care. Prepares families for discharge through are coordination and family education regarding patient needs. The successful completion of these duties contributes to the enhanced quality of cost effective discharge from Blank Children’s and Women’s Services.

Responsibilities
Care Coordination
• Monitors admission and continued stay of all patients regardless of level of care.
• Monitors avoidable days which are delays in treatment or timely patient discharge, patient care inefficiencies, and/or barriers to treatment and/or discharge.
• Updates patient file (PCP, insurance, etc.) to ensure timely discharge
• Collaborates with patient’s physician regarding discharge goals to develop discharge plans during daily physician team rounds
• Develops a discharge plan in with collaboration of the family and the multidisciplinary health care team.
• Collaborates with health care professionals internally and regionally to promote a smooth transition to home.
• Uses the nursing process to provide quality care.
• Coordinates appropriate medication, equipment, and/or home care services/supplies.
• Assists with follow-up appointments, activities, and schedules therapies.
• Meets with healthcare team to facilitate, coordinate, and evaluate patient’s discharge planning needs.
• Demonstrates ability to consistently meet the department workload and schedule.
• Utilizes a family-centered care approach (considering physical, psychosocial, spiritual, educational, safety, and related criteria) appropriate to the age of the patients served in the assigned service area(s).
• Prioritizes dissemination of information using available technology.
• Promptly completes documentation to ensure an accurate legal record.
• Demonstrates initiative to improve quality and customer service by striving to exceed customer expectations.
• Integrates and demonstrates FOCUS values and expectations for personal and service excellence to guide responsibilities; be open and objective to other’s views; give and welcome feedback; contribute to positive team goals; and put the success of the team above own interests.
• Practice reflects knowledge of HIPAA, DNV, OSHA and other federal/state regulatory agencies guiding healthcare.
• Demonstrates the knowledge that patient and workplace safety is a priority by preventing errors, accidents and infections and reporting potential or actual concerns immediately.
• Integrates the UnityPoint Health Des Moines Nursing Expectations of Performance in all aspects of care delivery and professional practice.
Qualifications

Education:

Experience:
| · Minimum of two years clinical pediatric experience working with multidisciplinary teams.
