Responsible for the organization, maintenance, and monitoring of an integrated health record system to ensure that confidentiality is maintained and that records are complete. Facilitates correction of chart information throughout all information management systems. Coordinates corrections needed with clinical staff, admin departments, and compliance. Responsible for ensuring all corrections, communication, attaching documents are completed in a timely manner in order to help support patient safety and coordination of care.

Responsibilities:

Departmental Support

  • Responsible for a wide variety of records processing functions such as phone coverage, processing record release requests, quick disclosure requests, sending records to storage or destruction of records after attached electronically and QA’d, retrieving records, performing QA checks to make sure records are complete, and sending incomplete documents to the clinical manager for completion.
  • Monitors and responds timely to all communication sent to Chart Correction team within the email box, and/or in-basket.
  • Responsible for locating and filing/scanning all medical records for patient visits according to office policies and procedures in a timely manner.
  • Corrects and resolves incorrect documentation issues within clinical systems such as multiple patients' information in one health record.
  • Acts as a liaison between the system Release of Information team and UnityPoint at Home for compliant record release
  • Makes independent decisions on how best to address documentation errors impacting patient safety and direct clinicians, and clinical departments on the necessary actions that must be taken to mitigate.
  • Works with staff to ensure smooth and coordinated correction of clinical information between patients and systems. Precision and accuracy are mandatory due to high risk to patient safety.
  • Maintains in depth understanding of data flow between the EHR, various repositories and ancillary systems. Attain a general understanding of site level information systems both active and historical (legacy).
  • Documents outcomes of the chart correction process according to organizational policies. Advise clinicians, HIM staff, and other customers on HIM procedures and assist in resolution of incorrect documents and documentation related issues.
  • Meets measures as determined and required for job productivity and performance improvement.
  • Assists with additional Health Information Management functions as requested.

  • Practices and reflects knowledge of HIPAA, TJC, DNV, OSHA and other federal/state regulatory agencies guiding healthcare.
  • Completes all annual education and competency requirements within the calendar year.
  • Is knowledgeable of hospital and department compliance requirements for federally funded healthcare programs (e.g. Medicare and Medicaid) regarding fraud, waste and abuse. Brings any questions or concerns regarding compliance to the immediate attention of hospital administrative staff. Takes appropriate action on concerns reported by department staff related to compliance.

Education:


High School or Vocational School graduate.


All staff are expected to participate in appropriate continuing education as may be requested and/or required by their immediate manager. In addition, staff are expected to accept personal responsibility for other educational activities to enhance job-related skills and abilities. All staff must attend mandatory educational programs.


Experience:


Previous medical office experience.


License(s)/Certification(s):


Valid driver’s license when driving any vehicle for work-related reasons.


Knowledge/Skills/Abilities:


  • Strong interpersonal skills.
  • Ability to work as a collaborative team member.
  • Ability to understand and apply guidelines, policies and procedures.


  • Knowledge of medical terminology.
