Upper Valley Services (UVS) is one of Vermont’s designated non-profit agencies serving people with developmental disabilities and adults who are eligible for Adult Family Care.
Upper Valley Services is seeking a compassionate provider to provide quality care, support and to develop a meaningful daily routine for a male individual who needs assistance with all aspects of personal care, mobility and daily living skills.
Individual enjoys children and pets, especially cats, and doing community activities, including bowling, using his tablet for movies or games and to video chat.
Provider must have accessible home or a willingness to make their home accessible and provide transportation & follow ups with medical appointments as well as leisure/community supports.
Reliable transportation, clean driving and background check is necessary.
CPR and First Aid and Safety Mechanics training provided.
Must live in the state of Vermont.
