Registered Nurse (RN) Case Managers oversee all medical services our clients receive which includes nursing, physical / occupational therapy, and home health aides. This begins with a comprehensive clinical assessment and creation of a specialized care plan, consistent with recommendations from the hospital, skilled nursing facility, and primary care physician. Direct nursing services include periodic health assessments, teaching, skilled intervention, care coordination, and medication management. All of this takes place in the client's home!
Utopia provides a full spectrum of nursing services such as Medical / Surgical, Wound Care, Orthopedic Care, and Behavioral Health. This position requires patience, compassion, and a desire to care for those who need it most, just like family. If providing care for seniors and clients with disabilities comes from your heart, this position is for you!
We are especially looking for:
- Behavioral Health experience
- At least 1 year of experience in a home care setting
- Bilingual in English and Spanish
No home care experience? That's ok! We have an assigned nurse educator who provides training as part of the on-boarding process.
