Building Name: CVMC - Woodridge Location Address: 142 Woodridge Drive, Berlin Vermont Regular Department: CVMC - Woodridge Social Workers Full Time Standard Hours: 40 Biweekly Scheduled Hours: 80 Shift: Day Primary Shift: 8:00 AM - 5:00 PM Weekend Needs: Other Salary Range: Min $24.94 Mid $31.18 Max $37.41 Recruiter: Alicia Sherman JOB DESCRIPTION: Under direct supervision of the Care Management and Life Enrichment Director the Social Worker provides social services to Woodridge residents and their families in order to attain or maintain the highest practicable physical, mental, and psychosocial well-being of each resident while adhering to state and CMS guidelines. Responsible for patient assessments, care planning and helping the patients and families adjust to their new surroundings. EDUCATION: Minimum of a Bachelor’s degree in a health related field including but not limited to sociology, counseling, human services, community wellness and psychology. One year of experience in a health care setting working directly with individuals. Must demonstrate knowledge of the issues and developmental stages associated with geriatric residents. Must be able to assess data related to age-specific needs and provide care as described in the department’s policies and procedures. EXPERIENCE A minimum of one year’s social work experience in a health care setting.
