Role: Social Worker (MSW)
Work Location: Woodbury, MN
Schedule: Monday–Friday, 8:00 AM to 5:00 PM (No weekends)
Territory Coverage:
Washington, Ramsey, Dakota, and parts of Hennepin Counties
Mileage reimbursed at $0.50/mile
Key Responsibilities:
- Provide supportive care to individuals, families, and groups in varied settings including homes, hospitals, and treatment centers.
- Assist clients with both temporary and chronic issues such as homelessness, domestic violence, unemployment, and substance abuse.
- Develop and implement individualized care plans based on psychosocial evaluations.
- Conduct individual, group, or family therapy sessions as needed.
- Coordinate discharge planning, documentation, and interdisciplinary collaboration.
- Engage in case management, crisis intervention, and advocacy.
Qualifications:
- Master’s degree in social work (MSW) – Required
- Hospice experience – Preferred
- Experience with HCHB – Helpful but not required
- Valid driver's license and willingness to travel within the designated region
