Valley Community Services Board is looking for a Co-Leader to join the ACT team. As a member of the ACT leadership team, the ACT Co-Leader works closely ACT team members to enhance psychiatric stability, quality of life, access to available resources, and appropriate use of medical services for individuals with serious mental illness (SMI).

Essential Functions
- Provides guidance, direction, and accountability for the ACT team in collaboration with the ACT Leader.
- Serves as an advocate for individuals with SMI.
- Provides direct services as needed including, but not limited to, case management, rehabilitation counseling, housing support, and vocational readiness assistance.
- Conducts crisis intervention.
- Develops and maintains productive professional relationships with community partners.
- Ensures compliance with applicable regulations and policies.
- Coordinates with Western State Hospital (WSH) Discharge Liaison for any current WSH inpatient clients being referred to ACT or currently on ACT census.
Salary : $20 - $1,500
