What are the responsibilities and job description for the Case Manager (FBMH) position at Valley Youth House?

Valley Youth House empowers and strengthens the lives of children, youth and families through inclusive programming that builds resilience and fosters growth and independence.

We are currently looking for a qualified applicant to fill the following position: FT - Case Manager ($18hr - $21hr).


Job Description:
Under the Senior Vice Presidents' and President's authority and the Program Supervisor's supervision, the Case Manager is responsible for contacts with and treatment services to youth and families; contact with line community agency personnel and administrative documentation of services provided.


Essential Functions:

  • Case Manager's time will be spent providing direct, in-person services according to established program time frames and protocols.
  • To provide effective case management services to clients in office, client homes, schools, and other settings as appropriate within the context of outreach services and according to treatment plans.
  • To complete and submit to the case file all clinical documentation, progress reports, and case recordings in a professional manner within established program time frames.
  • To complete the Home Based Safety Assessment and Plan according to program time frames and protocol. (Child Welfare Only).
  • To achieve stated treatment goals with assigned youth or families as identified by program expectations.
Key Functions:
  • To provide effective services in accordance with the program service model and expectations, including collaboration with members of the youth or family's treatment team.
  • To identify, link, and coordinate necessary services.
  • To provide regular ongoing case coordination and collateral contact with other community agencies also involved.
  • To participate in supervision with his/her/ their assigned supervisor according to program timeframes and protocols.
  • To meet at least one time per week with his/her/their assigned supervisor individually and/or as part of a team.
  • To successfully engage and maintain youth and families through the treatment process.
  • To follow the Incident Reporting procedure as required by federal, state, and agency practices and procedures.
  • To immediately report to the supervisor 1) any situation in which a child, family member, or another person is at risk of harm and/or 2) a death occurs in an open case or a case open at the agency within the last 90 days.
  • To immediately report to the supervisor any situation in which a client is threatening to sue Valley Youth House.
  • To provide crisis response to families in need including providing program on-call responsibilities which includes responding by phone within 15 minutes of the contact and going to the family home or community site if the situation can not be resolved by phone.
Other Duties:
  • To follow the policies, procedures, and rules stated in the agency personnel policies, Code of Business ethics, employee handbook, and program procedure manual.
  • The accurate and timely completion of personnel material (e.g. mileage reimbursement, time sheets, etc).
  • Support agency values and goals
  • To provide quality representation of the agency with other professional agencies or in court in a professional manner.
  • To provide quality representation of services with an individual in court when subpoenaed.
  • To represent the program in treatment conferences with other agencies in a professional manner.
  • Timely completion of training in accordance with program standards.
  • To complete other related tasks as appropriate or assigned.

Qualifications: A Case Manager shall be a mature person with sound judgment and represent an appropriate role model to youth and families. He, she or they must be an individual who has never been convicted of any child abuse offenses. A Bachelor's Degree is necessary. Training and experience in crisis intervention and one year experience in a CASSP system is mandatory for Case Managers in the Children's Behavioral Health Component.
